Currents and Waveform Descriptions
 
NMES is a symmetrical biphasic waveform with no interphase between the positive and the negative phase. The waveform has a short square pulse duration which the patient tolerates well. It penetrates and strongly stimulates nerve fibers in the skin and in the muscle. This waveform is also used in muscle stimulators for sport training. It is suitable therefore for muscle strengthening protocols.

TENS units are the most widely recognized of EMS machines because of their use for many years by the medical community in working with patients to manage pain.  Below are different TENS waveforms that each have a place in a practitioners arsenal when working with patients.

Symmetrical Biphasic waveform has a short square pulse duration. It penetrates and strongly stimulates nerve fibers in the skin and in the muscle. The patient tolerates the current well, thanks to its short pulse duration.

Asymmetrical Biphasic waveform has a short pulse duration. It penetrates and strongly stimulates nerve fibers in the skin and in the muscle. The patient tolerates the current well, thanks to its short pulse duration. Some medical professional, prefer it for its reported longer-lasting analgesic properties.
 
Monophasic waveform has a short square pulse duration only on the positive phase. It penetrates and strongly stimulates nerve fibers in the skin and muscle. The patient tolerates the current well thanks to its short pulse duration. The net charge delivered to the skin is not zero. Thanks again to its short duration, the net electrical charge is mall (24 μC at maximum intensity). The danger of electro-chemical burn is less significant at current values below maximum.

Burst waveform is a symmetrical biphasic waveform delivered in an extremely fast short
format. Because the pulse is relatively short, the waveform has a low skin load, making it suitable for applications requiring high intensities, such as in muscle strengthening protocols. The use for TENS burst is that many patients would not tolerate 80Hz - 120 Hz programs. Athletes subject themselves willingly to these programs, because they have a goal in mind.  Instead if one does it in short bursts, it is almost pleasing, like an energetic massage.  It may help to decontract a tense back, and other body parts in the short term. However, since the total work time is very short, it may not deliver lasting benefits.

Interferential Pre-modulated Current is a medium frequency waveform. The current is delivered through two poles, at medium frequencies. The maximum intensity of each waveform changes with a certain beat (modulation). There is no need for the traditional quadri-polar arrangement, thus making it easier for the professional to treat small skin areas where four poles would be cramped., and to repeat treatments consistently. 
 
Russian Currents or Kots, are delivered by a carrier waveform at 2500 Hz in bursts of 10 ms at 50 bps (bursts per second). Originally, because of the technology available at the time of their conception, the waveform was sinusoidal. Its author (Kots) claimed that this current produced maximal muscle strengthening effects without significant discomfort. With current technology it is possible to provide the same current with a rectangular waveform that thanks to its steep inception reduces the discomfort even further.
 
Microcurrent is a square waveform of very low intensity for a relatively long time resembling the electrical current generated by the human body. The current intensity is measured in μA rather than in mA, and the waveform pulse duration is measured in ms rather than μs. Because of its low intensity, it is applied by using a special set of cables. After each impulse the current reverses its polarity, to keep the electrical net charge at zero.
